Bans for pretty much everything are on a sliding scale, 7 days, 28 days, permanent. But they can reset after a while down to a lower level, also I may well give two 7 day bans in a row rather than a 7 then a 28.

I would permanently ban someone outright for (and I'm not necessarily limiting myself here, but off the top of my head) trying to hack the site or use exploits on it, or otherwise attempting to damage the site in any way.

If they succeeded in hacking or damaging the site then I'm sure they could find a way around the ban.
WHAT WOULD YOU DO THEN JAMES?!
Chaos would ensue, G-U would need a hero...

2-J is site owner, he could just remove the hacker's admin privileges. Or as a last resort, simply revert to a previous back-up.

The 'Pearl' account has been compromised and was almost certainly being used by Masked Mouse to make the last post.

Which raises a couple of issues, first of all Masked Mouse being banned shouldn't be using it to post, also, Pearl shouldn't have allowed others to access it to post in any case.

The Pearl account will remain deactivated until further notice in consequence. However, Pearl himself (the dude behind the account) is no longer banned and if he wishes we can discuss getting the account back to him, provided he doesn't allow its misuse. He can contact me on gorillaz-unofficial@hotmail.com to do this . Anyone who knows him, pass on the message if you could, thanks.

On this occasion, no. I think it must have been pretty obvious that ban evasion was unacceptable before, but now I've added another notice that clears it up, and I'll be strictly sticking to that in future.

Well, I don't really believe in IP bans. Anyone can use a proxy. Disabling accounts works, if someone who gets banned re-registers and doesn't act like they did before and never gives away their true/previous identity, well, the ban has served its purpose.
